WAIVER OF LIABILITY FOR GYM USE

I/We hereby understand and acknowledge that the training, programs and events held by Luxebody Baltimore may expose me to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.
I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.
After having read this waiver and knowing these facts, and in consideration of acceptance of my participation in the Luxebody Baltimore services, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE Luxebody Baltimore, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in Luxebody Baltimore training, programs and/or events.
By my signature I/We indicate that I/We have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms. I also agree to the gym rules below:
GYM RULES:
- You can cancel a class in your Member Portal over 24 hours in advance. You cannot cancel less than 24 hours before your class time.
- Please be on time for class, you cannot attend a class if you're more than 10 minutes late. Over 10 minutes late is a No-Show.
- No-Show fees will be charged to ALL class takers due to our smaller class size - this applies to members or classpack holders. Our no-show fee is a flat rate of $20.
- If you don't have boxing gloves, you can rent ours for $5.
- If you're a Member (bronze, silver, gold, VIP), you are entering into a minimum 3-month commitment. You can cancel by emailing us written notice 7 days before your next bill date. If you cancel before 3 months, you must pay a $100 termination fee.
- Upon arrival, check-in at the front desk and give the manager either your full name or 4-digit check in code. Check-in is very important, otherwise you will be charged a no-show fee. Do not skip!
